Provides advanced professional input to complex Nurse Case Management assignments/projects. Supports and provides direction to more junior professionals. Works autonomously, only requiring expert level technical support from others. Exercises judgment in the evaluation, selection, and adaptation of both standard and complex techniques and procedures. Utilizes in-depth professional knowledge and acumen to develop models and procedures, and monitor trends, within Nurse Case Management. RN and current unrestricted nursing license required.
Duties & Responsibilities:
Telephonic RN will conduct a 15 - 30 day follow-up triage phone call and report outcomes to healthplan/PCP as appropriate. Makes telephonic outreach calls to patients prior to TOC visit, as indicated Assists field RN with pre-visit screening tools and health information data collection, as indicated Responsible for post TOC visit activities (records requests, follow-up triage phone calls, PCP communication)
Reviews patient records for past and present health issues prior to the home visit. Performs telephonic medication reconciliation, as indicated
Completes required and pertinent clinical documentation of the TOC follow-up call within the electronic medical record that is accurate, high quality, and finished within the expected timeframe.
Collaborates with the field RN, primary care physician, the healthplan and other members of the Alegis health care team to ensure outstanding patient care is delivered.
Educates patients and/or power of attorney on chronic medical conditions, preventive care, and medication adherence.
Completes referrals to client case management when indicated. Educates the patient on the importance of following up with the Primary Care Physician. Contacts the PCP and/or other clinicians directly to ensure patient care continuity. Participates in clinical meetings as needed
