This role will participate in the performing of telephonic outreach to Cigna Medical Group (CMG) patients and will assist in coordinating clinical care needs for patients as they relate to strategic Performance Quality initiatives. This role will maintain complete and accurate documentation of outreach initiatives while maintaining patient confidentiality through coordination of services. This position ensures the delivery of quality outcomes through the careful attention to detail and use of problem solving skills in collaboration with CMG centers and resources. This role contributes to the organizations drive to improve the individuals experience of care; improve the quality of health, and works to reduce the cost of care across the population served.
