This role is for an administrative medical assistant. It is an officed-based, telephonic position. (No face-to-face interaction with patients or members)
This medical assistant will report directly to the care supervisor or Health Services Sr. Specialist. Medical assistant will provide administrative and triage support to the chronic care, complex care and primary care management programs and hold responsibility for order management and facilitation. MA will work to ensure that orders and provider requests are carried out. They will also be responsible for tracking the results of labs and diagnostics and the delivery of medical supplies/equipment to ensure timely execution and resulting.
Support of a live call que - management of inbound and outbound calls Thorough documentation of calls and actions taken in electronic medical record Generating orders through EMR and ensuring receipt by intended vendor or facility Ensure timely resulting of lab and diagnostic orders Ensure timely patient receipt of medical equipment/supplies Sorting and routing of incoming faxed documents Escalate patient care issues and barriers to care to appropriate members of the care team Assist providers and nurses in administrative functions as needed Market resource collection and organization ( venders, key contacts, and community programs)
Assist patients and providers in obtaining referrals from patients PCP. Completing the scheduling and facilitation of ordered consults Procurement of records and other needed information Participation in weekly rounds may be necessary depending on market supported Appropriate communication with all parties involved in patients care (provider, RN, SW, field MA, specialists, etc)
Notify care team of known hospitalizations and patient discharges. Special projects as requested
